To Renew for Student Teaching

Tuberculosis (TB) Tine Test Instructions

For Student Teaching:

Visit the Campus Clinic (#4350 for an appointment) in Spruce Hall for a TB Tine test. The Clinic will provide a portion of a three part form for you, and another for you to provide to the Field Experience Office - RC 204.


Visit your personal physician for a TB Tine test. Have your physician provide a prescription sheet, or office letter head including the following:

  • Patient name, social security number, and date of birth
  • Office name and location
  • Results (negative/positive) of TB Tine/Mantoux test
  • Doctor’s signature

Criminal Record Check Instructions (ACT 34)

On-line ACT 34 Criminal Record Check request: Pennsylvania Access to Criminal History (on-line with credit card) Print two!! Take copy to the Field Experience Office - 204 Retan Center

Pennsylvania FBI FIngerprint Clearance (ACT 114)

Identogo Fingerprint Instructions

In order to secure your PA FBI Fingerprint Clearance (Act 114), candidates need to enroll on-line through Identogo,  Candidates will have to enter the Service Code, 1KG6RT.  Key in SP-Mansfield and choose walk-in.  From here, candidates will follow the prompts and complete and print the registration form.  Next, candidates will get their fingerprints scanned at an approved Identogo site.  An appointment can be made during registration or walk-in appointments are also available.  Candidates will pay $22.60 (debit or credit only - no cash or money orders) at the time of the fingerprint scanning.   MU now has a fingerprint site:

After being scanned, candidates will receive an email providing access to their unofficial copy of the clearance.  DO NOT OPEN THE LINK UNLESS YOU CAN PRINT THE CLEARANCE IMMMEDIATELY!  Access to this clearance is a ONE TIME OPTION, and candidates cannot retrieve a physical copy of the clearance at another time.  Providing the UE ID number for access to the official copy will be the only option that a candidate has if he/she missed the one-time opportunity to print the unofficial copy.  In this case, students should submit their registration form to the Field Experience Office in lieu of the unofficial copy. 

Please note: Students seeking employment in states other than Pennsylvania will want to check individual state clearance requirements to be prepared for job searching after student teaching. (Ex: New York State requires the OSPRA clearance.)

Professional Liability Insurance

The Pennsylvania State Education Association (PSEA) (Note: PSEA membership includes professional liability insurance.

Be sure to print out 2 proof of membership forms – one for your records and one for the Field Experience Office - RC 204.

A note regarding professional liability insurance: Students shall be responsible for procuring professional liability insurance at their own expense. The limits of the policy shall be a minimum of $1,000,000.00 per claim and an aggregate of $3,000,000.00 per occurrence. This policy must remain in full force and effect for the duration of the practicum, field experience, or student teaching assignment.

Arrest/Conviction Report and Certification Form (ACT 24)

Pennsylvania Arrest/Conviction Report and Certification Form - Put your MU ID number in top right box, fill in your legal name and any previous names, check just one box in section 2 and provide specific information if needed, check just one box in section three, type in the current date, print out, sign, and take to the Field Experience Office - RC 204.

Statement of Understanding, Confidentiality, Professionalism, and Clearance Adherence (HBKH)

Read the Mansfield University Student Teaching Handbook, print Form 1 (Student Teacher Statement of Understanding), Form 2 (Student Teacher Statement of Confidentiality and Professionalism), and if necessary Form 3 (Student Teacher Statement of Work Understanding), read, sign and take to the Field Experience Office in RC 204.

Child Abuse Recognition and Reporting Training under PA (ACT 126)

Your ACT 126 must be less than 5 years old throughout your entire Student Teaching experience to be valid for Student Teaching. If you need to renew please take your original document along with a copy to the Field Experience Office, Retan Center, Room 204, prior to the established deadlines!
