Mansfield University is concerned about the safety and well-being of all students, faculty, and staff. In an effort to open the lines of communication across campus, the office of Student Conduct has have created this incident report form. This form provides an avenue for all faculty, staff, and students to report disturbing or suspicious activity to the Community Conduct Officer and Service Coordinator, Frank Crofchick. This information will be passed along to the Office of Student Conduct, University Housing, and/or the Behavior Assessment Team.
Incidents reported here may or may not include violations of the law or Code of Student Conduct. In many cases, this behavior may be classified as a mental health concern. Incidents may include (but are not limited to):
- Violations of the Student Conduct Code
- Violations of law
- Suicidal Ideation
- Abuse of alcohol or other drugs
- Classroom Disruptions
- Drastic change in behavior/hygiene
